Like most noise cancellation products, don’t rely on this for hearing protection. It’s good for reducing low frequency noise only - which might be a nuisance, but it’s less important from a hearing conservation standpoint.

That’s an over-estimate. About 40 dB is about the maximum attenuation anyone can achieve from a deeply inserted foam earplug or a very large earmuff. Your bones conduct sound at levels around 40 dB down from what your ears would hear, so there’s a physical limit there.

Shouldn’t be too hard. Different headphones or other devices would have a different impedance, which it could fairly easily detect. Some headphones also include a microphone and they have a TRRS connector, which the phone would need to detect - then it would know it’s not plugged in to a speaker, for instance.

You are correct that this is a small change. A 3-dB reduction is approximately half the sound power, but to the human ear it will be just about the smallest difference in sound levels you could reliably hear.

This sounds a lot like Confirmation Bias. Nice paper on a bottle has an effect similar to Placebo Effect, but even boxed wine can be well above average for its style and price point. But if it's a gift, get the nicer looking bottle anyway.
