Yes, torture porn or torture porn elements are defined by their realistic depictions of mutilation, if not out right sadism, I completely agree with you.

I sort of disagree with you, in the sense that I think Horror is a big genre and I think torture porn films are a subset of the horror genre. And just because you have increasingly realistic special effects doesn't mean you can't be psychological.
