Yeah, car bicycle collisions are often reported the same way. Often the absence of a helmet is reported on as if to suggest that the cyclist was just *asking* for that dump truck to right-hook them.

Certainly people should pay attention. But most car-pedestrian accidents aren’t because someone stepped in front of a car that didn’t have time to stop.

And how about drivers look up from their cell phones long enough to notice that there are pedestrians in marked crosswalks that they have a legal obligation to stop for?  

There is no such thing as an accident. Your first example is a clear example of fault on the part of the person who rear-ended the other vehicle - they were following too closely. In other instances, it may be road design that is at fault or negligent driver behavior. But there is always a reason for a crash. Shit…

Interestingly enough, I just took my first old person defensive driving course and the instructor was very adamant that we just stop using the word “accident” entirely and call it what it is, a “crash”.
