I have portable Irfanview on a Win7 machine. It has an issue that might relate to your problem or might not. In 'open with', there is Irfanview and Portable Irfanview in the list. So far, no trouble here, getting 'open with' to see my portable apps. I don't know if you did this, but in case not, point 'open with' directly at the exe for notepad++.

firefox -P in Windows is the shortest route I've seen to get to the profile manager, at least from the Run dialog box.
