We call those people "non-op," as in, they chose against having gender reassignment surgery for whatever (personal) reasons. In a way, you're right; surgery isn't necessary.....for everyone. But for others it is necessary. For some, it is life saving.

Because gender dysphoria is an actual condition that people suffer, and that is detrimental to the health of sufferers unless steps are taken to treat it. There is a reason why the attempted suicide rate for trans folks is roughly forty times the national average. Treatment is not an elective, it saves lives.
