It could be many things - UTIs are more common during pregnancy, as are kidney stones, burst ovarianc cyst, and on and on and on. Miscarriage is pretty much covered on tv - show blood.

With the 50 million things that can occur and/or go wrong during a pregnancy, at any point within the 9 months, it is perfectly plausible for her to have a sudden, intense, hit-the-floor pain.
