To answer number 1 - Cost and complexity... also, the turbo engine that they have doesn’t “just fit”. The engine in the BRZ sits much lower that it does in the Impreza. The turbo is too low and the exhaust is on the wrong side of the engine.
My F150 came with a code from the factory, but you’re supposed to change it, which I did.

Since this is a tail dragger it has a pretty limited view of what’s in front when not up to speed, and given that kayaks are generally low to the water, my guess is the pilot didn’t see the kayaker paddle into their path and wasn’t aware of them until they were leveling out.
