As for how they are getting along sans doctor: There's a medic. We see her in a few shots (long brown hair, usually pulled back). Plus, since there is a medical facility, I'm sure there's a nurse or two. The official doctor might be dead, but nurses can manage many things such as stab wounds, gunshot wounds, etc.
