The problem with the ACA is it helps out people making less than $40k per year but makes things more expensive (a lot) for people making over $40k without employer coverage. It has a lot of good parts but it's not too difficult to see why it's unpopular, even apart from all the GOP propaganda.

Similarly, the response would be the same as (c) above. They'd bring up middle east wars supported by Clinton to respond to Trump's rhetoric/shittiness involving muslims, bring up Clinton's rhetoric on deportation including children and unspecified "dangerous" undocumented immigrants.
