Gah, actually I think I was wrong – I had seen a tweet saying it was full body, but I just read an article that says it was a mix - prosthetics + an older woman double blended together with CGI.

(Just jumping in to say it wasn't a real old lady body - it was the actress who plays Melisandre in full body prosthetics.)

In the "inside the episode" Lena talked about letting Grace choose the lines for that scene. It is my impression that Grace is actually gender fluid (her family refers to her as Simon on Instagram) so it was at least done somewhat respectfully, or with the intention of being delicate about it.
