@Soaring: Yeah, because no smartphones (Droid X, Samsung Fascinate, Samsung Captivate (AT&T), Droid 2) are $199 (I'm working off literally the top of my head and a 7 second search of the Verizon website).
@thnikkamax: Sprint uses pretty much the same thing as AT&T as far as I know. High paying customers and primary lines on family accounts get upgrades as early as 12 months, as long as they pay $xx per month (that's AT&T program).
@Vone: AT&T still does something similar, except it's a $50 or $100 extra credit toward phones. It has never applied to iPhone however, and I assume that's Apple's doing since they control, excuse me, strongly suggest the price at which you sell their phone.

@JamesTrivettesHat: You do realize HSPA+ is multiple upgrades over the original UMTS "3G" network right? Plus AT&T is also building an entire LTE network. And AT&T also calls their network "the nation's fastest mobile broadband network." They haven't run around going "4G 4G 4G!!!1!," even if they plan to in the future.

@vel0city: Absolutely right. Although, as we all know, Verizon's "3G" is not as fast and doesn't have some of the features of AT&T's "3G" (simultaneous voice and data).
