I've seen it happen, it takes a long time. Like the customer has to be a regular, and is regularly awful. Then after 2-3 months when no one will serve them the manager may ask them to leave. Don't forget the customer is always right, especially when they are wrong.
Was the tip out on that table more than the tip? Where I worked in college the tip out was 4% of the sales, so it was easy to lose money in those situations.
