I think gore works, but I also think horror filmmakers think more gore is more scary, when it's probably just the opposite. More gore is more fantastical.
I think the Exorcist is similar to the Fly in that it's ultimately body horror—watching some kind of decay happening to you in ways that you can neither fully understand nor control. Except in the Exorcist, it's a mother watching it happen to her child, which adds a layer of parental guilt.
