With the US Bank prepaid visa I had, there was actually an online facility to register your name and address with the card. This wasn’t made clear in the card instructions but it turned out to be necessary to use it on Amazon.com.

One caveat to this. I did the same thing with a $100 prepaid visa-> Amazon conversion a week or so ago. Only problem is that the prepaid visa hadn’t been set up with my name and address. Took me three attempts to work out why the transaction wasn’t going through and fix the problem.
