I think that this is an issue currently being debated by the medical community, but as I understand, Biological Gender pertains to the characteristics of boy and girl- physical sex organs, and babies do have them.

I would say that for a baby, sex=gender. The gender defaults to the sex until they can express their feelings on the matter.
