Enjoyed seeing Bell and O'Neill, but when it became apparent that Bell is pregnant, I thought it had to be a flashback. Nope. She strikes me as too old to be fertile, although a very quick google search did not reveal the age of actress Catherine Curtin.
Did I miss the discussion about Nicky and Lorna? What to think of that? Nicky in the last episode secretly professed her love for Lorna, and overtly in this episode. All the while acknowledging what a nutjob Lorna is. And now Lorna is pregnant???

The lack of on-body weaponry is intentional - that's why it was such a big deal when Humphrey brought a gun on-site. I don't know prison policies about having weapons elsewhere (e.g., in a locked cabinet). I do agree that it's entirely illogical that no SWAT response has emerged.

To understand this, look to her flashback. At first I didn't understand her flashback (and like many, found the sorority stereotyping to be very lame). But now I see that this is her new sorority. She is bonding with these gals, not in a genuine way but because she is gaining some power and she is having fun.
I did not read "murderous rage." I read, high on unknown drugs, irritated, maybe jealous and extremely unpredictable because of said drugs.

Many of the doors are locked by the cheapo gym locker locks that Flores got from the commissary. Piscatella and his guys could kick those open with little effort.
