I think Xavier asking, “what did you do?” was him remembering that something bad happened, but assuming Logan must have done it. Logan didn’t have the heart to remind Charles he was a murderer, so he let Charles live with the false assumption.
I thought Xavier had a seizure and drove Logan mad, resulting in him killing everyone. But by the end it seemed like it was just the seizure alone that did it.
