Can please stop using the word they when a singular person is involved.  Use that driver or the driver. It's confusing as hell when you use a plural when a single person was involved.

The only aspect that may be the cause of the slippage is that many people tighten the quick release the wrong way. Instead of closing the handle of the release with the curve in, like palm of you hand, they close it with the curve outwards which does not apply the correct pressure on the collar.
