Correct. But the home and visitors benches are always set in each arena, like the dugouts are in baseball. I think each team shoots toward its own bench in the first half, then switch in the second half. Just like hockey, each team shoots opposite its bench twice in every game (or more, depending on number of OTs).

Think about line scores in baseball. The visitor is on top, home on bottom. But if you have limited vertical space, the home team moves up to the right of the visitor’s score. It’s probably easier to say “Carolina at Denver” than “Denver at home against Carolina”, so the visitors are listed first.

Depends on the league. I’ve seen Little League and AAU-level teams that bat 10, so there’s a pitcher and a DH in the same lineup. I’ve even seen games where the entire team cycles through the batting order, as many as 12-13 players.
