Here's my theory of what happened:
At first only Butters is in VR, everyone else is watching Butters in VR including Cartman who is messing with Butters by telling him to do stupid things at the virtual school. Butters gets stabbed by a hooker in VR and is therefore grounded in VR and in real life.
