Yes, there is literally nothing about practicing medicine that would make it difficult to stick to a strict schedule. Patients *never* take longer than planned to deal with or show up late and throw the schedule off or need to be wedged in last-minute due to serious issues or anything!

No, it wasn't, because it shows the distant future of a story that actually took place. A St. Elsewhere ending (like a Newhart ending) makes the entire series run someone's dream or fantasy.
