This reminds me of a story of a racer – I can’t remember who, but in my mind the setting was an 70s or 80s NASCAR ring – who crashed. When asked by a track-side reporter what happened, he replied with something along the lines of:
Sorry, but BMW didn’t make the nyloc that did its job perfectly. And regardless, quality-control, either for the hardware itself or the person/tool installing it.
My 1972 BMW had a large (22mm?) nyloc holding the steering wheel in place. It never loosened on its own in the 15 years I had that car.

This is an upper-midwest thing. Maybe it’s a thing in other places, too, but I’ve really noticed it living in Minneapolis (after having lived in New England and California).
