I wouldn’t say this is “bad QA,” but it’s definitely not “very good QA” either. If it were good QA, they would have had a checklist for every state within that encounter, and then they would have checked it. It sounds like they were just “winging it” for the battle, which isn’t what I would expect from “good QA.”
