So many times, the stories of these deaths are about routines being broken. They weren’t the one to normally take the kid in, they had to take a different route than they usually do, they had an early morning meeting and didn’t realize the other parent put the kid in the car before

Quite true! My husband and I chose to take each others’ last names — so now his Jones and my Smith have become Smith Jones (no hyphen). Autofill assumes a compound last name is just a misplaced middle and a last, so I still get traditional “Mrs. Jones” correspondence. C’est la vie.
