The Space Shuttle more or less flew autonomously and could have flown unmanned. NASA could never get over the possible stigma of flying an unmanned Shuttle, since they are and have been always on the defensive where the Manned programs are concerned.
I believe he got a clean bill of health from the doctors first after a long medical treatment program. NASA may be many things, but putting a man with a weak heart into orbit is not even on the radar (leaky SRBs, apparently, are.)
