The Opti-Spark ignition system on the LT1/LT4 C4 Corvette. It was an optically-fired ignition system located under the water pump. When the water pump inevitably failed, coolant would leak onto the Opti-Spark, which would kill the ignition.

Honorable mention to the 1-4 skip shift nonsense. At least they put the plug on…
