I think "the twist" is something imposed on him rather than something grounded in reality. The Sixth Sense defines him so much that people look at him through that lens, but imagine that it never came out. Would we still think that?

It is, in fact, a Japanese movie. Seems like this poster was a quickly made Korean export poster since they left the ominous Japanese text in the background. (Mask! Knife!)
