Yes, I agree ... the problem started with the employees (and probably not helped by vague guidelines at a guess). One would hope that upper management is worth their pay grade as determined by the speed of their damage control team.

IMO, the management element that has policies in place regarding ‘appropriate attire’ needs to review their operative guidelines for practical (and legal) enforcement. In the meantime, for purposes of damage control, there should be a formal apology to the wearer of the shirt in question and good faith reparation

Totally agree ... it is a known scenario which can be delicate even in the best of circumstances. My background is in teaching so the motivation and protocol set is functionally identical.
