If the 18 y/o never check into the flight, they were likely flagged as a no-show and their seat reassigned to a stand-by passenger. The stand-by passenger probably got to their assigned seat, saw a car seat occupying the seat instead, and asked a flight attendant what was going on.
One, the rules are primarily in place to prevent terrorists from using assumed identities. Two, since his 18-year-old son did not check-in, he was a “no-show” and essentially relinquished his “paid-for” seat.
