She is singing along with her own vocals on tape. It's always very obvious during her concerts (I mean, fair, she dances a lot on stage). You could also hear the backing track when she fell during the Brit Awards. She's just not the strongest vocalist, some days better than others. But she is a great performer.

I love that so many people always comment about Madonna lip syncing her performances, when most of the time, like this one, it couldn't be more obvious that she's singing live.
