I've been underweight at various points due to depression and anxiety, but I've never given anyone a hard time for asking how I lost weight. You can give them the real reason without being mean or delighting in an uncomfortable reaction.
I definitely record and weigh/measure everything I eat. It hasn't really caused me any problems though, so I don't mind too much. On saying that, I realise other people might think of it as a bit extreme.
I have OCD and I've found the food diary method not too bad on that front. Partly because I made it completely public and shared the link with friends. It means I can't go through a phase of eating too few calories without someone noticing and being critical.
Do you weigh everything you eat using food scales? Lots of people underestimate calories. It all adds up.

On the last 'Fat Shaming' article, I mentioned that I was advised to lose weight because I have PCOS. If I didn't, I'd have a higher than average risk of developing heart disease, diabetes and so on.

I know what you mean. I set my daily goal to 1,400, even though I actually aim for 1,200. I'm usually around 1,200, but it gives me enough to run over sometimes without feeling like I've had several bad days in a row.
