Not really; half the album was songs. I tended to focus on those because my first exposure to Leary was his MTV Unplugged special. Hard to imagine Bill Hicks rendering a Joe Jackson song a capella.

I think you're right about where it comes from, mostly. But there's something to be said for not masking life's challenges. Folklore often equates ugliness with evil, making it a plot point, so it's embedded in culture and hard to get away from. I remember reading about how the wife of the Too Many Cooks guy wasn't thrilled about his portrayal. He seemed to take it in stride though, happy to be working.

There's "Hollywood Ugly" which hardly means anything, and then there's roles where the whole point is to be shockingly hideous. I too wonder how that casting is handled. There's occasionally some controversy over local ads for extras which suggest they're looking for deformities.
A lot of those complaints only apply to a select few. Most millennials are dirt poor, just like most of the human race. Trophy validity is the least of their concerns.
