If I recall correctly, the book makes clear that the abuse escalated steadily over time, starting after their marriage. I don't think there's anything particular about their life now that causes it to get worse - it's just part of the normal trajectory of escalation.

Re: Ed, he's definitely not creepy in the book, but they do at one point say that while he's a good stepfather, he clearly never bonded with Abigail the way he did his own children, despite the fact that Abigail was very young when Ed and Madeline married. So, I wonder if what we are perceiving as creepiness was

They added Madeline's adultery and the problems in her marriage to the story in order to give Reese Witherspoon something juicier to work with, and it really does feel tacked on and unnecessary.
