Not sure about “could not see it.” I managed to not run over a black cow in the middle of unlit part of a highway at 10 pm in a rural area. The CRV certainly had reflectors that a cow does not. The cow, by the way, was resting after it appears a semi hit it.

I’m betting it landed on the Earth. It’s thin fiberglass over essentially a paper honeycomb core. Maybe the skin is 0.015 metal (3 sheets of paper thick) but more likely the fiberglass for dent resistance.

There’s no warning about when a bumper cover slides off.

Since the frame and drive are interlocking loops, to use the belt the frame needs a place for the belt to cut through, adding a structural break in continuity. With roller chain, the links can be separated and rejoined using a pin tool or a removable link allowing the use of a continuous frame. 

In this case it isn’t so much hindsight as that the airline used predictive maintenance software to look for anomalous events and act on them before things went south. In this case the software suggested they immediately look for the cause of the problem and the airline blew off their own recommendation.

The pilots f’d with it. They had a jam condition and could have landed with difficulty but decided to do some percussive maintenance to break it loose. They succeeded and it came completely lose. It’s a general caution to not attempt repairs of failures with unknown causes from the cockpit while in the air. 

Worse than that - they didn’t inspect the actual condition of the nut on the jackscrew and didn’t use the correct tool to measure wear of that nut. What didn’t help was that the redunancy was fictional - the backup nut saw exactly the same wear mechanism and was eroded just the same. All it did was allow the wear to…
