There haven’t been any such instances. The blockage is always at the exit door waiting for passengers that have already exited to clear the bottom of the slide. Those who died, died waiting due to conditions at the door and outside the plane, not from anyone grabbing a purse or a coat.

They had 8 minutes to gather their belongings and 10 more minutes to leave. Too many hyperventilate over getting items when the main blockage is the exit door and waiting for people to clear the escape chute to allow more to leave.
