I don't think the shop girl was confusing Joan with anyone but Joan used to work at Bonwit Teller in Season 3 as a manager.

In the first season, the make-over challenge was making over female MMA fighters/kickboxers. They never mentioned their sexuality but making over the "butch" woman has happened!
