The twist is that he's legitimately "the last man on Earth". It's all a Jacob's Ladder scenario where the entire show is a dream of him doing the things he always regretted not doing - in the moment before he crashes his vehicle into the boulder.

This kid better pay off or I am giving up on this show for good.
