I'm surprised Childrens Hospital's The Sultan's Finger LIVE didn't make this list. Gimmicky as hell but lasting for almost the entire episode surely it must be longer than most of these if not the longest.
And it uses the exact same hospital set as Scrubs - the elevators no longer in working order.
