I think both John and Lilly are 'sociopathic' - somehow, being turned undead has released their inner rage and sadism (like the vampires in Buffy), and made violence very easy for them.

The very poor are just persecuted for being poor, and any moral failings are just proof they deserve their condition.

Probably the problem being that actual period medicine would have been, "she's hysterical, let's masturbate her/cut her womb out/put her in freezing water/do some other crazy shit", which they showed in an earlier episode, and it doesn't really encourage the viewer's respect..
