I'm sure they didn't have every single beat planned (cause those can get messy). The bit with Root's hard-coding is more than a sufficient enough hint to show made it happen, but a little background on the nuances (even if it is one fan's interpretation), is fun to ponder.

Probably meant as a nod to dying heroes in general. Don't think that's Reese's real grave. Per the MPOV graphic last episode, it's early 2016, but the grave is marked 'November', & if they let it ride, it would've probably shown 2015. Ruling out that it's John's.
