This is funny because I think the FIRST flash game I ever played was a helicopter tunnel game where you clicked to make your vehicle rise and let go to let it fall. Aren't all games derivative in some way?

I think that's part of the problem with MMO's, everyone looks at it from the highest level of play. The endgame is endlessly scrutinized (and apparently no game has ever made enough endgame to satisfy its players) while the PVP has to be 100% balanced, no excuses.

All MMO's have their issues with always being online too. It's just that now there's so many people playing them the community is less apt to let them slide.
