It's a web clip icon. Just go to maps.google.com and add to home screen.

If you add a home screen icon from maps.google.com on an iOS device it will show up.

I think it is for apps that get access to the phone such as apps that use MAP.

Also note, all iPhone 4S models, regardless of carrier, are exactly the same. All iPhone 4S whether on AT&T, Verizon, Sprint, or anyone else has both CDMA and GSM chips inside. Except the AT&T model has CDMA disabled.

Yes, it works perfectly on iOS 6. Always in sync with all my devices, and when I send messages from my iPad it uses my phone number.
