I had the same question, and was reminded of people who are allergic to the vaccines (a massive minority, but still an important part of the conversation). They're the ones that "herd immunity" is supposed to cover - not the anti-vaxxers.

I lived in Rome for 3 years, and paid in cash or change for everything. Carried one of these for 3 years - everyone does over there. As a culture, the use of credit/debit cards isn't as prevalent there.
