Personally, I feel e-mail should be checked but a reply should not be expected. If it’s something important, I want to know regardless. My out-of-office reply always includes the e-mail of someone covering for me.

As a physician, I would echo all of Beth’s information above (at least with regard to the medical info, the part I am qualified to echo). This is particularly true regarding her description of chronic lyme disease and the internet conspiracy that is causing misinformation and harming so many people.
