You have to go your doctor and be tested for testosterone levels and even then your levels have to be low enough that TRT is an option. That is only one of the many benefits that is mentioned when marketed. Again, I've been through the treatments myself, it was not marketed to be that way and it is by no means a singular treatment for "old dudes to have their penises reinvigorated."
