Yeah, it’s confusing, I’ll give you that. The kind I had did require you to use spermicide around the edges (which made it hard to grip the little sucker) so I always thought the spermicide was to kill off the semen on the outside of it. But you do have to leave it in a while.

No, you put it up right next to the cervix. It’s really not that hard to get in even if it seems like it might be difficult based on the photo. There’s a lot of space up near the cervix. More space than there is in the tubular part of the vagina.
