After watching This Is the End, I'm totally fine with those two directing whatever movie they want. The fact they don't really want to do this unless they can make it stand apart from Indy is one of the reasons I could trust them.

Honestly, I was more worried that he'd use the scalpel on Ellie and not Joel. He's the last surgeon... and one of the last fireflies if he's going to die, why let Joel have his way? Plus they probably still could've harvested from Ellie for a bit after she died anyway.

I tried to leave the female nurse alive (mainly because she was already cowering by the time I aimed at her), but for some reason I didn't get the prompt to take Ellie. So.... I killed her and got the prompt XD
