That's not how I understand co-pays to work. An in-network provider agrees to take the allowed amount + the specified co-pay (which is based on the type/location of service, but is a fixed amount).

Seems like a terrible idea. In my experience the "elite" players are the biggest jerks in most MMOs.

@Product F(RED): 2 major GSM carriers who use only some of the same frequencies, so taking a phone made for one carrier to the other may or may not work for you. And you can't port a CDMA phone anywhere without a few difficult/maybe illegal steps.
