It exploded in the atmosphere so there will be many smaller fragments. They found a 6 meter crater in a nearby lake that they think was created by one of the fragments.

Also possible that he had in-ear protection.

Yep, it happened once with STS-51-F in 1985. They wound up in a lower than planned orbit. The scary thing is that they almost had an automatic shutdown of a second engine (which could have resulted in the loss of craft and crew) but the crew manualy prevented the shutdown. http://en.wikipedia.org/wiki/STS-51-F
