I tend to think Pathfinder handles LGBT issues well, and Starfinder is even better. Starfinder has a serum of sex shift for only 350 credits that lets you switch to any sexual characteristics you desire. That’s a far cry from the girdle of opposite gender as a cursed item!

This seems more to just be about having gay nonplayer characters in the official campaigns, not encouraging players to make their characters gay. That said, a DM can make the NPCs whatever they want, regardless of what they are in the campaign by default.
